Health Requirements

HEALTH REQUIREMENTS FOR ST. BARNABAS STUDENT-ATHLETES

SPORTS PHYSICALS - All students who participate in the St. Barnabas Parish Athletic Program (including intramural Saturday Soccer and Junior Basketball) must have either an annual Sports Physical or Medical Form on file that authorizes sports participation. Please submit a completed form to the school for each participant prior to participation in the first program activity (practice, game, or event) of the school year.

CONCUSSION PROTOCOL - Student health and safety is of utmost importance.  St. Barnabas follows the concussion protocol of the Illinois Elementary School Association (IESA).  Per our Athletic Board By-Laws, at the start of each school year, all students in Grades 3 - 8 must participate in the free-of-charge concussion screening.  Screenings are conducted each September during the school day, and results are accessible to parents/guardians through the screening evaluators.    Students and parents/guardians can learn more about the IESA Concussion Protocol HERE and the CDC Heads Up initiative HERE.

If you suspect that your son or daughter may have sustained a concussion, please contact Frank Markett at [email protected] or 630-989-1263 for help in the recovery process. We also ask you to alert their coach and Mrs. Sue Connolly in our Main Office.
